Servicing lasting longer than one shift |

Primary Authorized Employee
will leave his lock on the lock box for the duration of the lockout. |

Full time staff members
may leave their lock on the lock box provided that they will be available the next business day. |

Students
must remove their lock before leaving the building for any reason. |

Worker who applied lock is not available |

Person will be paged
If person responds he will be asked to remove his lock. |

Inspect Accelerator
If the accelerator tank is open, look inside and make sure that the person isn't inside. |

Remove Lock
The Primary Authorized Employee will use a master key to remove lock. |

|
 |
Notify Employee
The Primary Authorized Employee will notify the employee by email that his lock has been removed. |
|
 |
If Accelerator Tank is open
The attendant will be instructed not to permit entry if the employee returns. |
|
 |
If the Primary Authorized Employee is not available
The lab director, or acting director shall ensure that these provisions have been followed before removing the lock. |

Contractors are performing service or maintenance |
|
 |
LO/TO Coordination
The Primary Authorized Employee and the contractor will coordinate lockouts. |
|
 |
Information Exchange
Both must inform the other of their Lockout/Tagout procedures. |
|
 |
Inform Employees
The Primary Authorized Employee shall ensure that his personnel understand and comply with the contractors procedures. |
