 
Reporting Injuries and Incidents

Work-related injuries, illnesses, and incidents must be reported to your supervisor and your Department safety committee. |

Any injury or illness, e.g., a cut, shock, chemical exposure, sprained ankle,
strained back..., no matter how seemingly trivial, must be reported. |

|
If medical expenses of any kind are incurred, or lost work time,
a report must be filled out and signed WITHIN FOUR DAYS by the supervisor,
and then phoned to the Workers' Comp administrator. |

Any "incident" (generally regarded as an event that could have caused injury or illness or property damage) must also be reported. |
