Reporting Injuries and Incidents

1.  Work-related injuries, illnesses, and incidents must be reported to your supervisor and your Department safety committee.
  Any injury or illness, e.g., a cut, shock, chemical exposure, sprained ankle, strained back..., no matter how seemingly trivial, must be reported.
  If medical expenses of any kind are incurred, or lost work time, a report must be filled out and signed WITHIN FOUR DAYS by the supervisor, and then phoned to the Workers' Comp administrator.
2.  Any "incident" (generally regarded as an event that could have caused injury or illness or property damage) must also be reported.
