This is to be done by the PPE instructors. Download the certification form (an excel file) and make a copy for each person to be trained. This file uses macros, so you must have security settings that allow this. Fill out the identification information at the top of the page. Enter all rooms used by the trainee.

Select the hazard classes needed for the trainee. When you click on the checkbox next to each hazard class, an "*" will appear next to the PPE recommended for that hazard. Check all of the hazard classes that apply to the trainee. Print the form. During the training session, go over the hazard assessment for the areas where the trainee will be working indicating the PPE required for the activities the trainee will be doing (not necessarily everything on the hazard assessment). Finally have the trainee demonstrate the use of each type of PPE required.

If any of the other sections are used, indicate what the other hazard or PPE is on the line provided. Get all the signatures and place the completed forms in Ken Mueller's mailbox.
