Travel Reimbursement
Please fill in all highlighted areas in Section A of the [Form 21] that pertains to you, making sure that you use your home address. Print your email address clearly and sign your name in Section C. We will fill in all other areas using the receipts that you send.
If you are not a US Citizen it is imperative that you fill out all necessary information in Section A (making sure you add your email address). Also, if you are not a US Citizen, please include a copy of your passport & your I-94 along with your receipts. You will most likely be contacted via email concerning your reimbursement process.
